Donna was born in Kingsport, TN on March 25, 1975, and was the daughter of Howard Mann and Jennifer Clark.

In addition to her father, her son, Dakota Chase Addington, paternal grandparents, Allen and Laura Mann, maternal grandparents, Blanche Shuler and Worley Head, her father-in-law, Randy Addington, and uncle, Kenneth Head preceded her in death.

Donna was a loving wife, mother, daughter, and friend.

Surviving is her husband, Forrest Addington, Gate City, VA, son, Aiden Addington, Gate City, VA, mother and stepfather, Jennifer and Harold Clark, Kingsport, TN, sister, Lisa Shaffer, Kingsport, TN, mother-in-law, Betty Addington, brother-in-law, Chad Addington, uncle, Jeffery (Debbie) Head, KY, nephews, Joshua and Justin Shaffer, cousins, and many friends.

It was the wish of Donna to be cremated with no formal services at this time.
